How to Enroll

Before you enroll in a plan, you should think about the following:

  • What are your health care needs?
  • What can you afford?
  • What are your needs regarding access to providers (doctors)?
  • Do you want a reasonable limit on what you might have to pay for medical expenses each year?
  • How important is the quality of the plan?
  • Have you read and do you understand the Summary of Benefits?

There are six ways to enroll in a Generations Advantage plan:

  3. Martin's Point printed enrollment form, available to download from the link provided in the column to the right on this page.

    Simply fill out the form and mail it in the enclosed postage-paid envelope. You can also download the form in pdf format, print it, fill it out, and mail it to:

Martin’s Point Generations Advantage is a health plan with a Medicare contract offering HMO, HMO-POS, HMO SNP, Local and Regional PPO products. Enrollment in a Martin’s Point Generations Advantage plan depends on contract renewal.
